Over a long period of time (since mid-80s actually), the following has been developed and has served me well and the nativities too that seek readings for free or otherwise. The following may suit the requirements of this forum for seeking jyotish readings, hence I offer this in good faith:

1 Full Name: Not for indentification but for those who use namakshara etc in their analysis.
2 Gender:
3 Current location (City, Country):
4 Date of Birth:
5 Month of Birth:
6 Year of Birth:
7 Time of Birth:
8 Source of Time (birth certificate, family diary, other written record such as birthchart made at birth, parent's recall, rectified, etc):
9 Place of Birth: Name and coordinates if known and is a small town etc in which case the name of nearest large city and distance/direction should be provided. This is becoming less critical as better and freely available atlases are becoming commonplace.
10 Full date (YYYYMMDD) and brief description of one or two major events (marriage, childbirth, major accident, etc.):

Hmm... that is wise advice, given the current times! I suppose that poses a problem for public free gimme a reading areas on internet like this one.

The forums where I was giving readings were designed so that the information did not have to be posted publicly and was not revealed publicly, even on the free readings for the last many years. But here, the requests can be picked up by anyone and responded to.
