Actually not tricky but more like EXPENSIVE! For the nativity!!
Hence it was prescribed for Kings and the rich in *times* when astrology was supported by Royal Grants (parampara of raj jyotishis etc) and generally Kings and the Rich used to give gemstones in charity (like a moti ki mala or heeray ki angoothi) as remedial.
The practice without necessarily universal "classical" assent has in modern times (last hundred or two years) taken the convenient form of wearing the gemstones. Very practical from a worldly point of view! Gemstones tend to go up in value (unless you have been duped by a fly-by-nighter gemstone dealer! Yes Virginia, they do exist!!) so it is a win-win situation!
I have never received a straight answer from astrologers who habitually prescribe gemstones and so must have a repertoire/database of their efficacy! And not all do.
