Posted is my brother's birth details.

His ascendent is Pisces with Jupiter, Moon, Saturn and Mars in the 6th house of Leo. His 5th house has Rahu.
4th has Mercury & Sun.

For childbirth, I looked at his 5th house and having a natural malefic and co-lord of 12th, obstacles were sure. Also, the 5th lord Moon is in 6th, the house of obstacles re-affirming the obstacle part.
In addition to that, Moon is with Saturn and Mars, again, signifying delay and obstacles.
But, the main thing is that Moon is with Jupiter, the natural putrakaraka, lord of 5th house from Moon and lagna lord and Mars owning a trine and forming Chandra Mangala yoga, I thought all is not bad and childbirth is delayed and not denied.

He is currently running the Moon Mahadasha, where he had health problems signified by the Moon's position in the 6th house. But, Moon is the lord of 5th house and being with Jupiter, I was thinking childbirth is possible with the right antaradasha.

Transits wise, when Jupiter was in Scorpio, the aspect was on the 5th house of Cancer and currently, the transit is on Moon, the 5th lord and natal Jupiter too. Also, the double transit phenomenon is happening in the 6th house where the natal 5th lord is posited.

Can someone please guide me as to why childbirth did not happen inspite of all such positive influences?

putrakaraka (nat and chara) in 6th
barren planet in 5th house
6th lord with lord of putrapada in 12th from house of children.

November 2008 to may 2009 is a period when there will be indications of success in this direction, although I am more hopeful in the period between last week of May 2009 to Mid Jan 2011.

Parenthood will not be a positive or enjoyable experience overall.

Does not Panchamesha in 6th represent obstacles. I thought being in 8th or 12th would mean denial.

Thanks for your point "6th lord with lord of putrapada in 12th from house of children. " I had not observed that. I agree with you there.

I am surprised at you calling Rahu a barren planet. I remember a post from Rishiji where he mentioned Rahu represents multiplicity and Rahu's position in 5th is not detrimental.
So, I am a little confused here.

Again, I do not know why you thought parenthood would not be a good experience.

What is your opinion about the positive transits?

Like I mentioned, I am really interested in understanding the astro reasons and analysis.

Regarding the barren planet Rahu. I had mentioned that Rahu in the fifth can give mulltiple chidren or sometimes not any, if afflicted.
