Possible set of issues:

1. Chart matching: If there is nadi dosha / Yoni dosha then there chance of not getting child.

2. Afflicted 5th house

3. Afflicted putrakaraka Jupiter. , charakaraka in the chart.

4. aflicted first house causing impotency

5. Mainly karaka for marriage/ life after marriage venus is aflicted in females can cause death of children/ no children.

6. negative Beeja Kshetra. I still having trouble in calculation of this item.

I think this is the most sought after question after the Marriage Question. Many Couples cannot convert them to "Familiy" due to bad Planetory Combinations and more importantly Dasas. The Medical Astrology itself is a new Science which is getting newer additions with every new Case.

The important things to see can be,

1. The 5th House and 7th House Malefics create problems in conceiving in Women. If Planets like Rahu/Mars are in 5th House of Woman then there are chances of having serious Fibroids in Uterus or in some cases blocking of Fallopian Tube. It may also cause irregular Menstruations.

2.  In case of Males, Planet like Saturn/Rahu decreases the Sperm Count. This can also be checked if planets like Jupiter, Mars or Venus are Retrogade.

3. The Dasas of Slow Moving Planets (Jupiter or Saturn) cause major harm in Conceiving if these Planets are  Retrograde in Horoscope.

4. Planets like Rahu or Saturn in 5th House cause Delay in Conceiving due to Curse of Snake or Debt from Previous Births.

5. Malefics in 4th House can cause problem with Harmonal Levels as 4th House is related with Mind and Mental Peace. This is the 12 th House from 5th House hence its Maraka Sthan for the 5th House.
