I think the correct term, at least what I had heard was "bhumipatanam", literally meaning falling to the ground. This was suggested as the accurate time for recording birth in Kaliyuga. Others being showing of the head, breath/cry, etc.
Bhumipatanam as a specific 'event' would create problem for the astrologer because the child is never allowed to fall, even into a tray, it is received! More importantly, it would create trouble for the birthing team because they will get a class action suit on their hand so fast from irate parents that their heads will spin! Apart from it being a cruel act and immoral (letting a newborn drop to the ground!).
Just go with the obvious (at least in most cases). The gross moment of separate existence begins when the neonate takes its first birth independently even if the cord is still intact. The moment of autonomy is very telling!
It is true that there will be infants who must go on artificial respiration and some that breathe then fail and have to be resuscitated and supported by machines etc.

I once knew a bhikaran that never refused any 2 cents or even 1 cent! With patience over the week she had enough to buy two cups of coffee!
But seriously, if we do not find something in most, if not each and every posting or response that makes us curious, makes us look up a word or a concept, it would get so mind-numbingly boring that the life itself would get sucked out of Jyotish! And there have been many periods during which that is exactly what happened on the jyotish scene and progress stopped, pretty much. Then came the Kalki Awataar of internet! It was a lot of fun, that assignment to write about internet recently using hopefully a novel change from the typical midnight or noon charts used by many astrologers when the birthtime is not known. But I digress...

Just to get this thing clarified: exact birth time is when the baby take first breath even if attached to placenta?

Yes, i think so.
in womb, the child does not breath. As per elders who know, the sushmuna nadi remains open and it is closed once the breathing starts. When the sushmuna nadi is open, the past/present/future is known to the soul. Once it is closed and ida/pingala take over, the prarabdha karma takes over and starts giving the "fruits" to the body. till this happens, the soul will not "enjoy or suffer" since he, "the soul" is aware that he is not the body. If the soul knows and experiences that he is not the body then, karma has no meaning. Only Sri Suka, son of Ved Vyasa had this opportunity. He refused to come out of the womb till Sri Krishna assured that he will retain the "consciousness”. He was a born YOGI.
Hence the birth time can be the time when the physical body starts. Physical body starts when the ida/pingala become active and generates the "ego" that differentiates between the universal "I and individual "i". This happens when the breathing starts. Then the GAME (termed LEELA in our scriptures) starts.
